Please note: This form must be filled out by the visiting student only, and the visiting student must be present at the meeting.

I agree that while my meeting will be limited to 15 minutes, I can still email the Registered Dietitian to continue the conversation. Filling out this form does not guarantee an in-person meeting during my visit. Our Registered Dietitian will reach out to you with your confirmed date, time, and location. If they are unable to meet with you during your visit, our Registered Dietitian will reach out to you for a virtual conversation at a later date. This form is not for use for informing our culinary team of your dietary restrictions/preferences during your visit and allergens maybe present. Please inform a location manager or chef of your allergy during your visit.
